How our service works for your patients

 

  1. Referral Submission: Begin by obtaining a referral from the patient's GP or healthcare provider.
  2. Referral Processing: Once the referral is sent to specialists@connectedmedical.com.au , our admin team will process the referral within 1-2 business days and reach out to confirm appointment details.
  3. Payment Setup: A $200 deposit is required at the time of booking. The remaining balance of $405 is collected on the consultation day. After the appointment, the patient will receive a Medicare rebate of $444.90, leaving an out-of-pocket cost of $160.10. (Please note that these costs are for initial consultations only).
  4. Pre-Consultation Support: On the day of the consultation, our admin team will contact the patient 15 minutes before the scheduled appointment to assist with connecting to the telehealth platform.
  5. Consultation with our Specialist: During the video consultation our Consultant Geriatrician will review the patient’s medical history and provide a tailored care plan, including recommendations for follow-up care.
