Healthcare Without Borders: Bringing Specialist Care Closer to Home

Australia’s ageing population presents both challenges and opportunities for the healthcare system. With one in six Australians now aged 65 or older, and the number of those aged 85 and over projected to double to over one million by 2042, the demand for specialist geriatric and cardiology care is rising rapidly. Chronic diseases like cardiovascular conditions, arthritis, and dementia are becoming more prevalent, creating an urgent need for innovative healthcare solutions that ensure timely and equitable access for all Australians, regardless of location.

Recognising this growing demand, Connected Medical Solutions is expanding its telehealth services to include Geriatrician and Cardiology consultations. Our mission is to deliver healthcare without borders, bridging the divide between urban, rural, and remote areas. We believe that quality healthcare should not depend on your postcode, and our virtual specialist services are designed to bring expert care directly to patients’ homes or care facilities.

Why Timely Access to Specialist Care Matters

Timely access to specialists can significantly improve patient outcomes, particularly for older adults managing multiple chronic conditions. For example, cardiovascular diseases remain one of the leading causes of death in Australia, with 1.2 million hospitalizations related to heart conditions annually.

Similarly, age-related conditions like dementia and frailty require proactive management to support independence and quality of life.

By leveraging telehealth technology, we’re eliminating the barriers of distance and wait times, enabling faster diagnoses, tailored treatment plans, and seamless communication between patients, caregivers, and healthcare providers.

Who Benefits From These Services?

Our telehealth geriatrician and cardiology services are designed for:

  • Aged care facilities: Supporting residents with expert specialist care on-site.
  • Primary healthcare providers: Complementing GP-led care with specialised input.
  • Families and caregivers: Offering peace of mind and guidance on managing complex health issues.
  • Individuals in the community: Providing convenient access to specialist care without the need for travel.

How It Works

  1. Referral: A GP or healthcare provider sends a referral to specialists@connectedmedical.com.au.
  2. Scheduling: Our team contacts the patient or provider to arrange an appointment.
  3. Consultation: A video consultation is conducted, and a personalized management plan is developed.
  4. Follow-up: A detailed report is shared with the referring clinician for continuity of care.
